Connect North Link Worker (35 hours)
The main purpose of this role is to deliver health and wellbeing programmes as part of the trust health and wellbeing service. The post holder will work within a team to deliver individual and group-based interventions to support the needs of service users, families/ carers and staff, ensuring a high quality of service.
The programme aims to encourage and enable self-efficiency in the delivery of a social prescribing service. The post holder will support people to identify their holistic needs, co-create a personalised care plan and support connections to appropriate information and services.
The post holder may be required to support other health and wellbeing programmes.
The main responsibilities of the role are:
To identify, plan and deliver holistic individual needs led and/or structured support/ prevention interventions in line with licenced and other developed programmes, completing appropriate documentation.
To motivate and support service users, families/carers and staff through approved methods of communication and motivational influencing techniques to support holistic needs, change health and wellbeing behaviour and encourage self-efficacy to achieve improved outcomes and reduce health and wellbeing risks.

What Age NI can offer you:
Salary: £24,437 - £25,723 per annum
Hours of Work: 35 hours per week (5 days from 9:00am – 5:00pm) with hybrid working policy
Fixed Term Contract to 31 March 2026
